Applications for utility assistance (HEAP, PIPP, AT&T Lifeline and utility disconnection assistance referrals).
Advance Directives — education & form completion (living wills and durable power of attorney for health care).
Medical coverage review (medical coverage options, Medicare Part D and advantage plans, Medicaid, prescription assistance programs).
Support groups (Alzheimer’s Association and Caregivers support groups).
Any other questions regarding community resources.
Light refreshments will be included
